Our mission

Deep End Canada advocates for addressing health inequities in primary health care at individual, organizational, and policy levels through the collection and use of social determinants of health data and the sharing of ideas and projects.

About us

Primary Health Care at the Deep End Canada is one of the Deep End Project sites worldwide. What started as a network of physicians serving the 100 most deprived populations in Scotland, the Deep End Project now has coalitions in nine countries, including Japan, Australia, Ireland and now Canada.

Inspired by the tremendous work and advocacy done by other Deep End networks, we aim to bring together primary health care teams across Canada in a coalition to tackle the social determinants of health.

At Deep End Canada, we believe we can support more primary care sites to collect data routinely and systematically on the social determinants of health, share learnings across sites, and drive collective action to improve health equity.

Current members

Deep End Canada currently has 10 organizations from 7 provinces, representing around 25 clinics. Organizations are represented by health care providers, clinic administrators, policy and evaluation staff, researchers, and patient partners.

Deep End Canada is currently facilitated by the Upstream Lab, a non-profit research lab dedicated to improving the health and well-being of the population by addressing issues related to social determinants of health. Founded and directed by family physician and Public Health Specialist Dr. Andrew Pinto, Upstream Lab is based at MAP Centre for Urban Health Solutions, St. Michael’s Hospital in Toronto, Ontario and has strong ties to primary care across Canada.
